It damages you whenever a key is used to unlock a chest or a door. This includes opening the gates to Delvemore Dungeon for the first time by using the Dungeon Key.pngDungeon Key. (Unknown if the Key to the Halls.pngKey to the Halls has the same effect.) When used on doors that cost more than one key, such as when freeing Dibble, it will damage you for the same amount as using one key.

Haunted Locks will not trigger if the player opens a door with Bloodied Locks.pngBloodied Locks
